Kanza Library Card Application

Library Card Application Conditions: Patrons must live within 60 miles of the library. Must be 18 years of age or have a parent signature. A photo identification card, showing current address is required for patrons who wish to check out physical items from the library. Parent and/or guardian will be held responsible for all materials checked out underminor’s name, and will be responsible for any lost, not returned, or damaged materials. It is unlawful for anyone to refuse to return materials checked out from the library.
